Down syndrome (ICD-11 LD40.0) is a genetic condition present from birth, not something that develops at 12–18 months. It is almost always identified at or soon after birth — through newborn examination, characteristic physical features, and a confirming blood test (karyotype). So if your baby had a healthy newborn check and no diagnosis was raised, it is very unlikely to first appear now. What you can helpfully watch at this age is your toddler's overall development.

What is helpful to watch at 12–18 months

Rather than "Down syndrome signs," think about developmental milestones — useful for any toddler:
  • Movement — pulling to stand, cruising along furniture, beginning to walk by around 15–18 months
  • Communication — babbling with intent, a first few words, pointing to show you things
  • Understanding — responding to their name, following a simple instruction with a gesture
  • Connection — eye contact, shared smiles, copying simple actions like clapping

If several of these are clearly delayed, that is a reason for a general developmental check — not a reason to assume any one diagnosis. Low muscle tone (a "floppy" feel), feeding difficulty, or certain facial features noted earlier are things your paediatrician would already be tracking.
