  • Full name, date of birth and a contact number
  • A short note on your main concerns — what you've noticed, and when it began
  • Languages spoken at home

Any earlier records (only if you have them)

  • Previous developmental, speech, hearing or vision reports
  • A paediatrician or doctor's referral letter, if one was given
  • Birth and early-childhood history — pregnancy, delivery, early milestones, any hospital stays
  • A current list of medicines, allergies or ongoing medical conditions

Everyday picture

  • A typical day: sleep, meals, play, screen time and how your child communicates
  • What your child enjoys, and what tends to upset them
  • What you most hope to see change
