What to Bring to Your Child's First Appointment

Bring your child's health and immunisation records, any earlier reports or school notes, a written list of your concerns and questions, and a comfort item for your child. Your own everyday observations matter most — and if a document is missing, come anyway.

What to Bring to Your Child's First Appointment — Ask Pinnacle, the Child Development Kośa

The first appointment isn't a test you can fail — bringing a few simple things just helps us see your child clearly, faster.

In short

Bring your child's health and immunisation records, any earlier reports or school notes, a list of your questions and concerns, and a familiar comfort item for your little one. You don't need anything fancy — your own observations as a parent are some of the most valuable information you can offer. If a document is missing, come anyway; we work with what you have.

A simple checklist to pack

Your child's records
  • Health/vaccination card and any growth charts
  • Birth and early-development notes if you have them (pregnancy, delivery, milestones)
  • Previous reports — hearing tests, vision checks, paediatrician letters, earlier therapy or assessment summaries

Your own notes

  • A short list of what you've noticed at home — speech, play, movement, eating, sleep, behaviour
  • When you first noticed it, and whether it's changed
  • Your top 2–3 questions, written down so nothing gets forgotten
  • School or daycare feedback, if any

For your child's comfort

  • A favourite toy, book or snack
  • A spare set of clothes for younger children
  • Anything that helps your child feel settled in a new place

Practical bits

  • ID and contact details
  • A note of any medicines your child takes
  • If possible, bring the parent who spends the most time with the child — their everyday observations matter

What to expect

This first visit is a friendly conversation and a gentle look at how your child plays, communicates and moves — not a pass-or-fail exam. Tell us your worries plainly; parent concern is one of the most reliable early signals there is. If you forget a document, that's completely fine — we'll note what's available and help you gather the rest.

The Pinnacle way

At Pinnacle Blooms Network, a clinical AbilityScore® and any diagnosis are formed only at a centre, under qualified clinician care — never from a checklist or a single visit. Across 70+ centres in 4 states, with 700+ therapists and 4.95 lakh+ families served, our team makes that first appointment calm and clear. Trusted sources

Guidance on preparing for a developmental visit draws on parent-facing resources from the American Academy of Pediatrics (HealthyChildren.org) and the CDC's "Learn the Signs. Act Early." programme, both of which encourage parents to bring records and written observations to early appointments.

Next step — book your child's first assessment, or message the Pinnacle team on WhatsApp at +91 91001 81181 with any questions before you arrive.

This is general information, not a diagnosis — a clinical AbilityScore® and any diagnosis are formed only at a Pinnacle Blooms Network centre under qualified clinician care.

What to watch

If your child has had any recent loss of skills, a high fever with unusual movements, or a sudden change in behaviour, mention it at the very start — some concerns need prompt medical attention rather than a routine visit.

Try this at home

Keep a notes app or small diary for a week before the visit — jot down anything that surprises or worries you about play, talk, eating or sleep. Real examples help us far more than trying to remember on the day.

Frequently asked

What if I don't have my child's old reports?

Come anyway. We'll work with whatever you have and note what's missing. Your own observations as a parent are often the most useful information at a first visit, and any documents can follow later.

Do I need a referral from a doctor to come?

No referral is required to book a first developmental assessment. If you do have a paediatrician's letter or earlier reports, bring them along, as they give helpful background.

Should my child come prepared or fasting?

There's no fasting or special preparation needed. Bring a familiar toy and a snack so your child feels comfortable, and let them be themselves — we want to see how they naturally play and respond.

Who should come with the child?

Ideally the parent or carer who spends the most time with the child, because their daily observations are invaluable. If both parents can attend, that's a bonus, but it's not essential.
