- Languages spoken at home
Any earlier records (only if you have them)
- Previous developmental, speech, hearing or vision reports
- A paediatrician or doctor's referral letter, if one was given
- Birth and early-childhood history — pregnancy, delivery, early milestones, any hospital stays
- A current list of medicines, allergies or ongoing medical conditions

- A typical day: sleep, meals, play, screen time and how your child communicates
- What your child enjoys, and what tends to upset them
- What you most hope to see change
