Modified Checklist for Autism in Toddlers, Revised with Follow-Up

M-CHAT-R/F: What It Is and What It Assesses

The M-CHAT-R/F is a free, parent-completed screening questionnaire for toddlers aged about 16–30 months that asks simple questions about play, pointing, eye contact, gestures and responding to others. It is a two-step tool — a checklist followed by a structured follow-up interview — designed to flag children who may benefit from a fuller developmental assessment. It is a screen, not a diagnosis; many children who screen positive are not autistic, and the AAP recommends routine autism screening at the 18- and 24-month visits.

M-CHAT-R/F: A Gentle First Step, Not a Diagnosis — Ask Pinnacle, the Child Development Kośa

A short, friendly questionnaire that helps spot whether a toddler might benefit from a closer look at their development — that is the M-CHAT-R/F.

In short

The Modified Checklist for Autism in Toddlers, Revised with Follow-Up (M-CHAT-R/F) is a widely used, freely available screening questionnaire for toddlers aged roughly 16 to 30 months. It is completed by a parent or carer and uses simple yes/no questions about how a child plays, communicates, points, makes eye contact and responds to others. Importantly, it is a screen, not a diagnosis — it simply flags when a child may benefit from a fuller developmental check.

What it assesses and how it works

The M-CHAT-R/F asks about everyday behaviours that, taken together, give early clues to social communication and play development. It looks at things such as whether a child points to show you something interesting, follows your gaze or your point, responds to their name, enjoys back-and-forth play like peek-a-boo, makes eye contact, imitates others and uses gestures.

It has two parts. First, the parent answers around twenty questions (the "Revised" checklist). If the answers suggest some areas worth exploring, a trained person works through the Follow-Up interview — a structured set of clarifying questions about the specific items that were flagged. This two-step design reduces unnecessary worry by giving families a chance to describe what they actually see, before deciding whether a full assessment is the right next step. A result is described as lower, medium or higher likelihood of benefiting from further evaluation — never as a diagnosis of autism.

When a screen leads to next steps

A screen that flags some areas is not a verdict — many children who screen positive are not autistic, and a screen is only a prompt to look more closely with qualified clinicians. The American Academy of Pediatrics recommends routine autism screening at the 18- and 24-month well-child visits, which is why tools like the M-CHAT-R/F are so common. If your child's screen suggests further review — or if you simply have questions about how your child communicates and plays — a comprehensive developmental assessment is the right next step.

The Pinnacle way

A screening questionnaire like the M-CHAT-R/F is a starting point, never a diagnosis — a clinical AbilityScore®, our clinician-administered structured assessment, and any diagnosis are formed only at a Pinnacle Blooms Network centre under qualified clinician care, never from an app or form. Where support helps, our team draws on individualised plans that may include speech therapy and other developmental supports, mapped to your child's unique strengths.

Next step — If your toddler's screen flagged some areas, or you'd simply like clarity on how they play and communicate, book a developmental assessment to understand the whole picture.

What to watch

In toddlers, watch for whether your child points to show you things, follows your point or gaze, responds to their name, makes eye contact, enjoys back-and-forth play, imitates you and uses gestures — these are the everyday behaviours the M-CHAT-R/F asks about.

Try this at home

Build natural moments to notice and encourage these skills — point at birds or buses and say 'look!', play peek-a-boo and clapping games, and pause to see if your child points back or copies you. Shared, playful attention is the heart of what the screen looks at.

Frequently asked

What age is the M-CHAT-R/F for?

It is designed for toddlers aged roughly 16 to 30 months. The American Academy of Pediatrics recommends routine autism screening at the 18- and 24-month well-child visits, which is when it is often used.

Does a positive M-CHAT-R/F mean my child has autism?

No. It is a screening tool, not a diagnosis. A positive result simply means it would be helpful to look more closely. Many children who screen positive turn out not to be autistic. Only a qualified clinician can form a diagnosis after a full assessment.

Who fills in the M-CHAT-R/F?

A parent or carer completes the first checklist of yes/no questions. If some areas are flagged, a trained person works through the structured Follow-Up interview to clarify what you actually see in your child.

What does the M-CHAT-R/F actually look at?

It asks about social communication and play — for example whether your child points to show interest, follows your point or gaze, responds to their name, makes eye contact, imitates you, uses gestures and enjoys back-and-forth play.
