Modified Checklist for Autism in Toddlers, Revised with Follow-Up

At What Age Is the M-CHAT-R/F Used?

The M-CHAT-R/F is a parent-completed autism screening questionnaire used for children aged 16 to 30 months (about 1½ to 2½ years), most often at the 18- and 24-month checks. It is a screening tool, not a diagnosis — it has two parts, including a brief Follow-Up step that clarifies borderline answers before any referral. A result places a child in a likelihood band for further review, never a label.

A quick, gentle questionnaire designed for toddlerhood — the M-CHAT-R/F has a clear age window for a reason.

In short

The M-CHAT-R/F (Modified Checklist for Autism in Toddlers, Revised with Follow-Up) is a parent-completed screening tool used for children aged 16 to 30 months — roughly between 1½ and 2½ years. It is most often given at the routine 18-month and 24-month developmental checks. It is a screening questionnaire, not a diagnosis — it simply helps decide whether a child would benefit from a closer developmental look.

Why this age window

The 16–30 month range is chosen because early signs of differences in social communication, play and interaction often become noticeable around this time, while a child's everyday skills are developing rapidly. Screening here means support can begin early, when it tends to help most. The tool has two parts: a short set of yes/no questions you answer about your child's everyday behaviour, and a brief Follow-Up set of questions used only when the first part suggests a closer look is worthwhile. The Follow-Up step is what gives the "R/F" its name — it reduces unnecessary worry by clarifying borderline answers before any onward referral.

What a result means

A screen result is never a label. It places a child into a low, medium or higher likelihood band for needing further assessment. Many children who screen positive turn out to be developing typically — and a few who screen negative may still benefit from review if a parent or doctor has ongoing concerns. If you have worries at any age, including before 16 months or after 30 months, that is always reason enough to ask for a developmental check; the M-CHAT-R/F is simply one helpful tool within a wider picture.

The Pinnacle way

A screening questionnaire is a starting point, not a diagnosis — a clinical AbilityScore® and any diagnosis are formed only at a Pinnacle Blooms Network centre, under qualified clinician care, never from an app or form. We use tools like the M-CHAT-R/F alongside structured clinician observation, and where helpful build an individualised plan that may draw on speech therapy and other supports.

Next step — If your child is between 16 and 30 months, or you have any developmental concern, book a developmental review to understand their strengths and start any helpful support early.

What to watch

Differences in social interaction, pointing or showing things to share interest, response to name, eye contact, pretend play or spoken language as your toddler approaches 18–24 months.

Try this at home

At your child's 18- and 24-month visits, ask your doctor whether an M-CHAT-R/F screen is due — and answer it honestly based on your child's everyday behaviour, not their best day.

Frequently asked

What age is the M-CHAT-R/F for?

It is used for children aged 16 to 30 months (about 1½ to 2½ years), most commonly at the routine 18-month and 24-month developmental checks.

Is the M-CHAT-R/F a diagnosis of autism?

No. It is a screening questionnaire that flags whether a closer developmental look may help. A diagnosis is made only by qualified clinicians after a full assessment.

What does the 'Follow-Up' part do?

The Follow-Up is a short set of clarifying questions used only when the first part suggests a closer look. It reduces unnecessary referrals by checking borderline answers.

My child is older than 30 months — what should I do?

The M-CHAT-R/F is designed for under-30-months, but concern at any age is reason enough to seek a developmental review using age-appropriate tools.
