What is actually appropriate to observe at two

Rather than hunting for "ADHD signs", notice the broad developmental picture — these are the things that genuinely matter at 24–36 months:
  • Communication — using and combining words, following simple instructions, pointing to share interest
  • Social connection — back-and-forth play, looking to you for reassurance, copying what you do
  • Play — pretend play (feeding a doll, stirring a pot), curiosity, exploring with purpose
  • Movement & settling — walking, climbing, and being able to settle with comfort and routine

High energy, a short attention span, running off, and big emotions are expected at two and are not, on their own, signs of ADHD. What is worth mentioning to a clinician is the wider pattern — for example, if speech, social engagement or play seem delayed alongside the restlessness, or if a previously gained skill is lost.

When ADHD assessment becomes meaningful

Under ICD-11 (6A05), ADHD describes inattention and/or hyperactivity-impulsivity that is persistent across settings (home and nursery/school) and out of step with a child's developmental level — a judgement that simply cannot be made reliably in a toddler. Most reliable assessment happens from around school-entry age. Girls in particular may show quieter, more inattentive patterns later, so a calm watch-and-share approach now serves her well.
