Down Syndrome
Worried About Down Syndrome at 12–18 Months?
Down syndrome is present from birth and is almost always identified at or soon after birth through examination and genetic testing — not something that newly appears at 12–18 months. At this age, watch general milestones, and see your paediatrician for any delay. Diagnosis is confirmed only by a clinician.
If a worry about Down syndrome has settled in your mind, let's meet it gently and clearly — because what you're noticing at this age has a hopeful, practical next step.
In short
Down syndrome (ICD-11 LD40.0) is a genetic condition present from birth, not something that develops at 12–18 months. It is almost always identified at or soon after birth — through newborn examination, characteristic physical features, and a confirming blood test (karyotype). So if your baby had a healthy newborn check and no diagnosis was raised, it is very unlikely to first appear now. What you can helpfully watch at this age is your toddler's overall development. What is helpful to watch at 12–18 months
Rather than "Down syndrome signs," think about developmental milestones — useful for any toddler:- Movement — pulling to stand, cruising along furniture, beginning to walk by around 15–18 months
- Communication — babbling with intent, a first few words, pointing to show you things
- Understanding — responding to their name, following a simple instruction with a gesture
- Connection — eye contact, shared smiles, copying simple actions like clapping
If several of these are clearly delayed, that is a reason for a general developmental check — not a reason to assume any one diagnosis. Low muscle tone (a "floppy" feel), feeding difficulty, or certain facial features noted earlier are things your paediatrician would already be tracking.
The Pinnacle way
A diagnosis of Down syndrome is confirmed by a paediatrician through clinical examination and genetic testing — never from an online list, and never from worry alone. At Pinnacle, no diagnosis or clinical AbilityScore® is formed except in person at a Pinnacle Blooms Network centre, under qualified clinician care. If your child does have Down syndrome, early support through special education and therapy helps them flourish — that is our whole purpose.Trusted sources

What to watch
Seek a developmental check if your toddler isn't bearing weight on legs, has no babbling or words, doesn't respond to their name, makes little eye contact, or shows persistent low muscle tone. These point to a general assessment, not one specific diagnosis.
Try this at home
Build face-to-face play into your day — clapping games, peek-a-boo, naming objects you point to together. This both supports development and gives you a natural sense of how your toddler is connecting and responding.

Frequently asked
Can Down syndrome appear at 12–18 months if it wasn't found at birth?
It is very unlikely. Down syndrome is a genetic condition present from conception and is almost always identified at or soon after birth through newborn examination and a confirming blood test. If no concern was raised earlier, it is improbable to first emerge now.
What should I actually watch for at this age?
Focus on general milestones: pulling to stand and cruising, babbling and first words, responding to name, pointing, eye contact and copying actions. If several seem clearly delayed, ask your paediatrician for a developmental check.
How is Down syndrome confirmed?
A paediatrician confirms it through clinical examination and a genetic (karyotype) blood test. It is never diagnosed from an online checklist or from worry alone.
If my child does have Down syndrome, what helps?
Early support makes a real difference. Special education, speech and developmental therapy, and family guidance help children with Down syndrome learn, connect and thrive.
