It sets clear, achievable targets across the areas that matter for your child (such as speech, play, movement, learning or daily skills), names who will help and how often, and is reviewed regularly so it grows as your child grows. The whole point is that your child's plan looks like no one else's.

What goes into the plan

  • A clear picture of your child today — built from a structured clinician-led assessment of strengths and areas of need, plus your own observations as the parent who knows them best.
  • Goals that matter to your family — practical, meaningful targets written in plain language (for example, asking for water using words, sitting for a short play activity, managing buttons independently) rather than vague aims.
  • The right mix of support — which therapies, how often, and how they work together — for instance speech therapy, occupational therapy or behavioural support, chosen for your child.
  • Your role at home — small, repeatable everyday strategies, because steady practice in real life is where the biggest gains happen.
  • Review points — regular check-ins where progress is measured and the plan is adjusted, so it stays one step ahead of your child.

A good plan is collaborative, flexible and always built with you — never handed down. You should understand every goal and feel able to ask why it is there.

When the plan is made — and revisited

An individualised plan is created after the initial assessment, and then revisited at regular intervals or whenever your child reaches a goal, plateaus, or their needs change.
