What is an individualised therapy plan for my child? — Pinnacle Ask answer card with a short explanation and QR link

YOUR QUESTION. A CLEARER NEXT STEP.

What is an individualised therapy plan for my child?

THE SHORT ANSWER

An individualised therapy plan is a personalised, written roadmap for your child's development — built around their unique strengths, needs and goals rather than a one-size-fits-all programme. It sets clear, meaningful targets, names the right mix of support and how often, includes your role at home, and is reviewed regularly so it grows with your child. AbilityScore® is part of Pinnacle’s developmental assessment. Diagnosis, where needed, requires an appropriately qualified healthcare professional.

An individualised therapy plan is your child's own roadmap — built around who they are today, and where you both want to go next.

In short

An individualised therapy plan is a personalised, written roadmap for your child's development — designed around their unique strengths, needs and goals rather than a one-size-fits-all programme. It sets clear, achievable targets across the areas that matter for your child (such as speech, play, movement, learning or daily skills), names who will help and how often, and is reviewed regularly so it grows as your child grows. The whole point is that your child's plan looks like no one else's.

What goes into the plan

  • A clear picture of your child today — built from a structured clinician-led assessment of strengths and areas of need, plus your own observations as the parent who knows them best.
  • Goals that matter to your family — practical, meaningful targets written in plain language (for example, asking for water using words, sitting for a short play activity, managing buttons independently) rather than vague aims.
  • The right mix of support — which therapies, how often, and how they work together — for instance speech therapy, occupational therapy or behavioural support, chosen for your child.
  • Your role at home — small, repeatable everyday strategies, because steady practice in real life is where the biggest gains happen.
  • Review points — regular check-ins where progress is measured and the plan is adjusted, so it stays one step ahead of your child.

A good plan is collaborative, flexible and always built with you — never handed down. You should understand every goal and feel able to ask why it is there.

When the plan is made — and revisited

An individualised plan is created after the initial assessment, and then revisited at regular intervals or whenever your child reaches a goal, plateaus, or their needs change. If you ever feel a goal no longer fits your child or your family's life, that is exactly the right moment to ask for a review.

The Pinnacle way

AbilityScore® is part of Pinnacle’s developmental assessment. Diagnosis, where needed, requires an appropriately qualified healthcare professional — never from an app or online form. From that clinician-led structured assessment we build your child's individualised plan together with you, and refine it through speech therapy and the wider team. Explore how our therapy support is shaped around each child.

CONNECT THE ANSWER TO YOUR CHILD’S DAY

Something to notice. Something to discuss.

What to notice

Watch whether your child's goals still feel meaningful and achievable for your family, whether you understand each one, and whether progress is being reviewed and the plan updated. If a goal no longer fits or progress has stalled, that is the right time to ask for a review.

In everyday life

Pick one goal from the plan and weave it into a daily routine your child already enjoys — practising during play, bath time or snack feels natural and adds up far faster than a separate 'practice session'.

Bring your observations and questions to your child’s professional. Choose activities that suit your child’s comfort, abilities and agreed plan.

Questions families ask

How is my child's individualised plan created?

It begins with a structured, clinician-led assessment of your child's strengths and needs, combined with your own observations as the parent. From there, clear goals and the right mix of support are agreed together with you, and written into a plan you can understand and follow.

How often is the plan reviewed?

The plan is revisited at regular intervals, and also whenever your child reaches a goal, plateaus, or their needs change. You can ask for a review any time a goal no longer fits your child or your family's life.

Can I be involved in setting the goals?

Yes — a good individualised plan is built with you, not handed to you. The most meaningful goals come from what matters in your family's daily life, and you should understand the reason for every target in the plan.

Will my child have the same plan as other children?

No. The whole point of an individualised plan is that it is shaped around your child's unique profile, goals and pace — so it should look like no one else's.
