What causes Down syndrome in children?

Down syndrome is caused by an extra copy of chromosome 21 (trisomy 21), present from conception. In about 95% of cases it arises from a random event during egg or sperm formation; less often from translocation or mosaicism. When a parent first hears the words 'Down syndrome', the very next question is almost always: did I cause this? The answer is clear, kind, and freeing — no, you did not.

In short

Down syndrome is caused by the presence of an extra copy of chromosome 21 — which is why it is also called trisomy 21. Every cell normally carries 23 pairs of chromosomes; in Down syndrome there is an additional 21st chromosome, and that extra genetic material shapes how a child grows and develops. It happens at conception, before a baby is born, and it is nobody's fault — not caused by anything a parent did, ate, thought or felt during pregnancy.

How the extra chromosome happens

There are three ways the extra chromosome 21 appears, and a simple genetic test (karyotype) tells which one:
  • Trisomy 21 (about 95% of cases) — every cell has three copies of chromosome 21 instead of two. This arises from a random event called nondisjunction, when chromosomes don't separate as they should during the formation of the egg or sperm.
  • Translocation (about 3–4%) — an extra part of chromosome 21 attaches to another chromosome. This is the one type that can sometimes be inherited from a parent who carries it silently.
  • Mosaicism (about 1–2%) — only some cells carry the extra chromosome, often leading to more variable features.

The single best-known factor that raises the chance is higher maternal age at conception — but Down syndrome occurs across all ages and backgrounds, and most babies with Down syndrome are born to younger mothers simply because more babies overall are. It is not caused by lifestyle, diet, stress or anything that happened during pregnancy.

What this means for your child

Down syndrome is recognised at or around birth, and a child with it will reach milestones — often along their own timeline and with the right early support. Speech, motor skills, learning and independence all respond beautifully to early, structured therapy. The cause is fixed; the future is not.
