Childhood Autism Rating Scale, 2nd ed.
What is the CARS-2 and what does it assess?
The Childhood Autism Rating Scale, Second Edition (CARS-2) is a clinician-administered rating tool that helps trained professionals identify and describe behaviours associated with autism. It is not a diagnosis or a self-test — it is one structured instrument used alongside observation, developmental history and parent input. The CARS-2 looks across areas such as relating to people, communication, emotional and sensory responses, and body and object use, with forms suited to different ages and ability levels.
A structured way for trained clinicians to bring careful observation and parent insight together into a clear, shared picture of autism-related behaviours — that is the CARS-2.
In short
The Childhood Autism Rating Scale, Second Edition (CARS-2) is a clinician-administered rating tool that helps trained professionals identify and describe behaviours associated with autism spectrum differences. It does not replace a diagnosis — it is one structured instrument that a clinician uses, alongside observation, developmental history and parent input, to build an overall picture. The CARS-2 looks across several everyday areas such as relating to people, communication, emotional response, body and object use, and sensory reactions. What the CARS-2 assesses
The CARS-2 is built to capture how a child responds and behaves across a range of real-life situations rather than testing any single skill. A trained clinician rates a number of behavioural areas, which typically include relating to other people, imitation, emotional response, body use, use of objects, adaptation to change, visual and listening responses, taste–smell–touch responses, fear or anxiety, verbal and non-verbal communication, activity level, intellectual consistency, and an overall clinical impression. The second edition added forms suited to different ages and ability levels — including a version designed for verbally fluent, higher-functioning individuals — and a questionnaire that gathers parent and caregiver observations. This breadth matters: autism shows itself differently in different children, and a tool that looks across many areas at once gives a fuller, fairer picture than any one observation alone. Crucially, the CARS-2 is a rating scale, not a verdict — its purpose is to organise careful observation so that a clinician can decide what further support or assessment a child may need. When it is used
The CARS-2 is one of several instruments a clinician may draw on when a family or teacher has noticed differences in a child's communication, play, social connection or sensory responses, and wants to understand them better. It is always interpreted by a qualified professional within a wider assessment — never used as a stand-alone, self-administered test or a label. If you have noticed such differences in your child, a developmental review is the right first step. The Pinnacle way
This is general information, not a diagnosis — a clinical AbilityScore® and any diagnosis are formed only at a Pinnacle Blooms Network centre, under qualified clinician care, never from an app or form. Where helpful, our clinicians may use structured tools such as the CARS-2 within a full assessment that looks at the whole child, then build an individualised plan that may draw on autism therapy and related supports.Trusted sources

What to watch
Differences in how a child connects with people, communicates verbally or non-verbally, responds emotionally, plays with objects, adapts to change, or reacts to sounds, sights, touch and other sensory input.
Try this at home
If you are curious about your child's development, keep a simple note over a couple of weeks of how they connect, communicate and play — these everyday observations help a clinician far more than a single appointment snapshot.

Frequently asked
Is the CARS-2 a diagnosis of autism?
No. The CARS-2 is a structured rating scale that a trained clinician uses to organise observations of autism-related behaviours. A diagnosis is made by a qualified clinician using this alongside developmental history, direct observation and parent input.
Can I do the CARS-2 myself at home?
No. The CARS-2 is administered and interpreted by trained professionals. There is a questionnaire that gathers parent and caregiver observations, but the ratings and their meaning are decided by a clinician within a full assessment.
What areas does the CARS-2 look at?
It rates behaviours across areas such as relating to people, communication, emotional and sensory responses, body and object use, adaptation to change, and an overall clinical impression — giving a broad picture rather than testing one skill.
At what age can the CARS-2 be used?
The second edition includes forms suited to different ages and ability levels, including a version for verbally fluent, higher-functioning individuals. A clinician chooses the appropriate form for your child during assessment.
