Global Developmental Delay

Early signs of Global Developmental Delay on a home visit

On a home visit, screen for Global Developmental Delay using the RBSK 4 Ds — delay or difficulty across movement, speech, social skills and daily thinking. Refer promptly when a child is behind in two or more areas, or has lost any skill once held, alongside a hearing and vision check.

Spotting Global Developmental Delay early — Ask Pinnacle, the Child Development Kośa

Every home visit is a quiet chance to notice the child who is learning a little differently — and your eyes are often the first the family will ever meet.

In short

Global Developmental Delay (GDD) means a young child is significantly behind across two or more areas — movement, speech, thinking, or social skills — compared with peers. On a home visit, look for the RBSK 4 Ds in everyday play and family report; persistent delay in more than one area, or any loss of skills, warrants prompt referral for assessment.

Signs to look for during a home visit

Motor (sit, stand, walk, grasp)
  • Floppy or very stiff body; poor head control past 4 months
  • Not sitting by 9 months, not walking by 18 months
  • Not reaching for or holding objects at the expected age

Speech & hearing

  • No babble by 9–12 months; no single words by 16 months
  • Does not turn to name or everyday sounds

Social & play

  • Little eye contact, smiling or interest in faces
  • Not playing simple give-and-take or pretend games

Daily skills & thinking

  • Not feeding self, pointing or following simple instructions at the expected age

Always act on

  • Any loss of skills the child once had, at any age
  • Persistent parental concern — family report is a sensitive early signal

When to refer

A child need not meet full criteria for you to refer. Delay across two or more domains, or any regression, is enough to route the child onward — alongside a hearing and vision check. "Wait and see" is not appropriate; early action gives the best window for support. Use your RBSK referral pathway to a District Early Intervention Centre or a developmental team.

The Pinnacle way

A clinical AbilityScore® — a clinician-administered structured assessment — and any diagnosis are formed only at a Pinnacle Blooms Network centre under qualified clinician care. What to watch

Escalate to prompt referral on any loss of previously held skills, or when delay spans two or more domains with parental concern — these need action, not monitoring. Always pair with a hearing and vision check.

Try this at home

Quick home check: does the child turn to their name, reach for a toy, and copy a simple wave or smile? Any two weak, with family worry, is enough to refer.

Frequently asked

What does Global Developmental Delay mean?

It means a young child is significantly behind across two or more developmental areas — such as movement, speech, thinking and social skills — compared with peers of the same age. It is identified in children under five, while a fuller diagnostic picture forms as the child grows.

How is GDD different from a delay in just one area?

A delay in only one area — say, late walking with everything else on track — is usually monitored on its own. GDD describes delay across two or more domains, which is why it warrants a coordinated assessment and early support.

Should I refer if I am unsure?

Yes. A child need not meet full criteria for you to refer. Persistent delay in two or more areas, any loss of skills, or strong parental concern all justify onward referral plus a hearing and vision check.
