Fetal Alcohol Spectrum Disorder

Early Signs of Fetal Alcohol Spectrum Disorder at 4 Years

In a 4-year-old, early signs of FASD can include smaller growth, certain subtle facial features, hyperactivity and attention difficulties, speech and language delays, and trouble with self-control, routines and social cues — especially where there was prenatal alcohol exposure. These are signs to observe and discuss with a clinician, not to diagnose at home, and early support helps.

Early Signs of FASD in a 4-Year-Old — Ask Pinnacle, the Child Development Kośa

If alcohol was part of a pregnancy, you may be watching your 4-year-old closely — so what's worth a gentle, informed second look?

In short

Fetal Alcohol Spectrum Disorder (FASD) follows prenatal alcohol exposure and can show in a 4-year-old as a mix of growth, facial, learning and behaviour differences — for example smaller stature, certain facial features, hyperactivity, difficulty with attention and self-control, speech and language delays, and trouble with everyday routines. These are signs to observe and discuss with a clinician, never to diagnose at home. With early understanding and support, many children make meaningful progress.

Early signs to watch at 4 years

Growth and physical features
  • Smaller height or weight than expected for age
  • Subtle facial features sometimes linked with FASD (such as a smooth area between nose and upper lip, a thin upper lip, or smaller eye openings)
  • Smaller head size noted on routine checks

Learning, attention and thinking

  • Difficulty paying attention, easily distracted, very high activity levels
  • Trouble with memory, following multi-step instructions, or grasping cause and effect
  • Speech and language that lags behind same-age peers

Behaviour and daily living

  • Difficulty with self-control, big emotional swings, or trouble calming down
  • Challenges with routines, transitions and understanding social cues
  • Poor sense of danger or difficulty learning from consequences

No single sign confirms FASD, and many of these overlap with other developmental differences. What matters is the overall pattern, a known history of prenatal alcohol exposure, and how much daily life and learning are affected.

When to seek a check

If alcohol was used during pregnancy — even small or occasional amounts — and you notice delays in speech, attention, behaviour or growth, a developmental check is worthwhile. FASD is often best supported when recognised early, before school demands increase. Because its signs overlap with ADHD, language delay and learning differences, a thoughtful assessment looks at the whole child rather than any one feature.

What to watch

Watch for a pattern of slower growth, certain facial features, high activity and poor attention, speech delay, and difficulty with self-control and routines — especially with a history of prenatal alcohol exposure. Seek a developmental check when several appear together and affect daily life.

Try this at home

Keep routines simple and predictable, with one instruction at a time and clear visual reminders. Short, calm transitions and lots of praise for small wins help a child with attention and regulation challenges feel secure and succeed.

Frequently asked

Can FASD be diagnosed at 4 years old?

FASD signs can often be recognised by age 4, particularly where there was prenatal alcohol exposure. However, a diagnosis is never made at home — it is formed by qualified clinicians who assess growth, features, development and history together. A developmental screen is a good first step.

What if only a little alcohol was used in pregnancy?

There is no known fully safe amount of alcohol in pregnancy, and effects can vary. If alcohol was used at all and you notice developmental concerns, it is worth a gentle, non-judgemental developmental check so your child can get any support early.

How is FASD different from ADHD at this age?

Attention and hyperactivity overlap with ADHD, but FASD often also involves growth, facial features, learning and language differences linked to prenatal alcohol exposure. A whole-child assessment helps tell them apart and guides the right support.
