In short

Fetal Alcohol Spectrum Disorder (FASD) follows prenatal alcohol exposure and can show in a 4-year-old as a mix of growth, facial, learning and behaviour differences — for example smaller stature, certain facial features, hyperactivity, difficulty with attention and self-control, speech and language delays, and trouble with everyday routines. These are signs to observe and discuss with a clinician, never to diagnose at home. With early understanding and support, many children make meaningful progress.

Early signs to watch at 4 years

Growth and physical features

• Smaller height or weight than expected for age
• Subtle facial features sometimes linked with FASD (such as a smooth area between nose and upper lip, a thin upper lip, or smaller eye openings)
• Smaller head size noted on routine checks

Learning, attention and thinking

• Difficulty paying attention, easily distracted, very high activity levels
• Trouble with memory, following multi-step instructions, or grasping cause and effect
• Speech and language that lags behind same-age peers

Behaviour and daily living

• Difficulty with self-control, big emotional swings, or trouble calming down
• Challenges with routines, transitions and understanding social cues
• Poor sense of danger or difficulty learning from consequences

No single sign confirms FASD, and many of these overlap with other developmental differences. What matters is the overall pattern, a known history of prenatal alcohol exposure, and how much daily life and learning are affected.

When to seek a check

If alcohol was used during pregnancy — even small or occasional amounts — and you notice delays in speech, attention, behaviour or growth, a developmental check is worthwhile. FASD is often best supported when recognised early, before school demands increase. Because its signs overlap with ADHD, language delay and learning differences, a thoughtful assessment looks at the whole child rather than any one feature.
