Down syndrome is almost always identified at or very near birth — through a blood test (karyotype) and recognisable physical features — not discovered fresh at age five. If your child was checked as a newborn and no diagnosis was made, it is extremely unlikely to appear now. What you can act on at five is any worry about how your child is learning, speaking, or keeping pace with peers — and that is worth a developmental check regardless of cause.

What this means at five

Down syndrome (ICD-11 LD40.0) is a chromosomal condition present from conception, typically confirmed in the first days of life. By age five, a child with Down syndrome would already be known to have it. So the real question behind your worry is usually: is my child developing as I'd hope? Those are different, and answerable.

Watch for patterns rather than single moments — speech that stays very behind same-age peers, difficulty following two-step instructions, trouble with self-care like dressing or toileting, or struggling to keep up in kindergarten. These point to a developmental review, not a Down syndrome diagnosis.

When to check

If learning, language or play feels persistently behind other children, book a general developmental assessment. If your child has never had a paediatric review, a one-time check is sensible and reassuring. A teacher's gentle concern is also a good reason to ask.
