A routine visit is a quiet opportunity — a frontline worker who knows what to listen for can catch a communication delay long before it becomes a worry.

In short

During routine home and PHC visits, check whether the child is communicating in age-expected ways — listening and responding to sound, babbling, using gestures, saying first words, and combining words. The simple rule: watch for no babble or gesture by 12 months, no single words by 16 months, and no two-word phrases by 24 months — and always act on any loss of skills already gained.

Milestones to check by age

By 6 months — startles or quietens to sound, makes cooing or gurgling noises, smiles and turns toward a familiar voice.

By 9–12 months — babbles in strings ("ba-ba-da"), responds to their own name, points or waves, looks where you point.

By 18 months — uses several single words, follows a simple instruction ("give me"), points to show you something.

By 24 months — joins two words ("more milk"), is understood by family, points to body parts or pictures when named.

By 3 years — speaks in short sentences, is understood by people outside the family, answers simple questions.

Always note — any loss of words, babble or gestures the child once had, or a strong parental concern. Refer in parallel for a hearing check, since hearing loss often shows first as a speech delay.
