The Ages & Stages Questionnaires, Third Edition (ASQ-3) is a parent-completed developmental screening tool used for children from 1 month to 66 months of age (about 5½ years). It comes as a series of age-specific questionnaires — 21 in total — so the questions always match your child's stage, from early infancy right up to the start of school. It is a screening tool that flags whether a child may benefit from a closer look — not a diagnosis.

How the age range works

Rather than one form for every child, ASQ-3 uses separate questionnaires at set age intervals — for example around 2, 4, 6, 9, 12 months and onward — so the developmental questions stay relevant as your child grows. Each questionnaire asks you, the parent, about everyday skills across five areas: communication, gross motor, fine motor, problem solving, and personal-social. Because parents see their child every day in familiar settings, your observations make this a wonderfully practical first step. The intervals are closer together in the early months, when development moves quickly, and a little wider as children grow. The full span runs from 1 month up to 66 months, which neatly covers the early-childhood window when timely support makes the biggest difference.

What a screen tells you — and what it doesn't

ASQ-3 is designed to be a screen, not a diagnosis. A result simply tells you whether your child is developing comfortably for their age or whether some areas might benefit from a closer professional look. A score that suggests follow-up is an invitation to understand more — never a label, and never a cause for alarm. Many children flagged by a screen go on to thrive, sometimes with a little targeted support, sometimes with none at all.
