Understanding the risk

Not every premature baby will face developmental challenges — many catch up beautifully, especially babies born only a few weeks early ("late preterm"). Risk is best thought of as a gradient:
  • Late preterm (34–36 weeks) — usually mild risk; most develop typically with routine monitoring.
  • Very preterm (28–31 weeks) and extremely preterm (under 28 weeks) — higher chance of differences in motor skills, speech and language, attention, learning and coordination.
  • A low birth weight, time in neonatal intensive care, or breathing difficulties at birth can add to the picture.

A helpful habit for parents: use your baby's corrected age (age from the due date, not the birth date) when looking at milestones in the first two years — an early baby is simply working to a different clock.

When to seek a check

Most premature babies are enrolled in a follow-up programme — do attend those reviews. Beyond that, seek a developmental check if, by corrected age, your child is not meeting movement, communication or play milestones; if you notice stiffness or floppiness, persistent feeding trouble, or limited eye contact and response to sound; or simply if something feels off to you. Early observation is reassurance, not alarm.
