What the ADOS-2 assesses

During an ADOS-2 session, the clinician sets up playful, naturalistic activities — bubbles, toys, picture books, conversation, or a story, depending on age and language level — that create gentle opportunities for social communication to emerge. The clinician then observes how the child or person responds: how they share enjoyment, make eye contact and gestures, take turns, use language to communicate, respond to their name, engage in imaginative or pretend play, and manage transitions. The schedule is organised into modules chosen by the person's age and spoken-language level — from a toddler module for very young children with few or no words, through to modules for verbally fluent older children, adolescents and adults. This lets the same careful framework flex to suit a chatty eight-year-old or a one-year-old just beginning to babble. The aim is to see social communication in action rather than only ask about it.

What it is — and is not

The ADOS-2 is a clinician-administered observation tool, not a stand-alone diagnosis. A thoughtful assessment combines it with a detailed developmental history (often using a parent interview), an understanding of how your child does in everyday settings, and the clinician's overall judgement. An ADOS-2 session is designed to feel like guided play — most children find it engaging rather than stressful — and there are no 'right' or 'wrong' answers for a parent to prepare. It simply helps a skilled team understand how your child experiences and shares their social world.
