Selective Mutism is not something we look for, or worry about, in a 12-to-18-month-old. It is a condition where a child who can speak comfortably in some settings (usually home) consistently does not speak in specific social situations (such as nursery or with unfamiliar people) — and that pattern can only be recognised once a child has reliable spoken language and spends regular time in social settings where speech is expected, typically from around 3 years onward. At 12–18 months your little one is still building first words, and quietness around strangers is normal, healthy caution. What matters at this age is steady communication progress, not silence in particular places.

What is actually worth watching at 12–18 months

Rather than mutism, this age is about the early building blocks of communication. Reassuring signs to look for:
  • Babbling and jargon — strings of sounds with the rhythm of talking
  • First words emerging (often around 12 months, give or take)
  • Pointing, waving, showing — sharing attention with you
  • Responding to their name and simple words like "no" or "bye"
  • Understanding more than they say — following "give me the ball"
  • Warming up to familiar people, even if shy with new ones at first

Being quiet, clingy or watchful with strangers is completely typical now — it is wariness, not a disorder. Selective Mutism (ICD-11 6B06) is recognised much later, when a child clearly speaks freely in one setting yet not in another over time.

A general developmental check is wise — not for mutism, but for communication — if by around 18 months your child is not babbling, not gesturing, not responding to their name, has no words at all, or has lost skills they once had. These point to early speech-language support, which is highly effective when started gently and early. The question of Selective Mutism becomes meaningful only nearer 3 years, if a talking child stays consistently silent in particular places for a month or more.
