1. Register online. Visit the Government of India UDID portal (swavlambancard.gov.in) and create an application. You can also apply offline through a designated district hospital or CMO office.2. 3. Submit and receive an assessment appointment. The application is routed to a notified government hospital with an authorised medical authority for your child's condition.
4. Attend the medical assessment. A specialist or medical board examines the child, reviews reports, and determines the category and percentage of disability per the standardised guidelines for that condition. For developmental and intellectual conditions, this may involve a paediatrician, psychologist or psychiatrist.
5. Certificate and UDID card issued. Once approved, the disability certificate is generated and the Unique Disability ID card is dispatched or made available for download. The certificate may be permanent or have a review date depending on the condition.
6. Use it for entitlements. The certificate unlocks scholarships, school accommodations, travel concessions, scheme benefits and reservations.
