ADHD vs Down Syndrome

ADHD vs Down Syndrome in Young Children

ADHD and Down syndrome are very different. Down syndrome is a genetic condition caused by an extra chromosome 21, present from birth and usually recognised early, affecting physical features, growth and learning. ADHD is a neurodevelopmental pattern of attention, activity and impulse control that emerges over the early-to-school years, with no birth test or physical sign. One is identified by genes, the other by behaviour observed over time — and the two can sometimes co-occur.

Two very different conditions, often confused — yet each child is wonderfully capable in their own way.

In short

ADHD (attention-deficit/hyperactivity disorder) and Down syndrome are quite distinct. Down syndrome is a genetic condition, present from birth, caused by an extra copy of chromosome 21 — it is usually recognised at or soon after birth and affects physical features, growth and learning. ADHD is a neurodevelopmental pattern of attention, activity and impulse regulation that emerges and is recognised over the early-to-school years, with no single physical sign or birth test. One is identified by genes; the other by behaviour observed over time.

How they differ in young children

Down syndrome is confirmed through a chromosome test, often soon after birth, and is associated with characteristic facial features, lower muscle tone (hypotonia), and a developmental pace that is steadier and slower across milestones. Health checks for the heart, hearing and thyroid are part of routine care.

ADHD, by contrast, is not visible on any scan or blood test. It is a pattern — sustained difficulty with attention, restlessness or impulsivity that is beyond what's usual for a child's age and shows up across home and preschool. It is rarely assessed meaningfully before the preschool years, because high activity and short attention are normal in toddlers. Importantly, the two can co-occur — a child with Down syndrome may also show ADHD-type attention patterns.

When to seek a review

If you have any concern about your child's development, growth, attention or learning, a developmental check brings clarity and the right support early.

The Pinnacle way

This is general information, not a diagnosis — a clinical AbilityScore® and any diagnosis are formed only at a Pinnacle Blooms Network centre, under qualified clinician care, never from an app or form. Our team understands both ADHD and genetic conditions, building individualised plans that may draw on occupational therapy and other supports.

Trusted sources

WHO ICD-11 on neurodevelopmental conditions; CDC and the American Academy of Pediatrics (HealthyChildren) on ADHD and Down syndrome in children.

Next step — If you'd like clarity on your child's development, book a developmental review to understand their strengths and any helpful support.

What to watch

Down syndrome is usually identified at birth via a chromosome test and is linked to characteristic features and low muscle tone; ADHD shows as age-beyond restlessness, short attention or impulsivity across home and preschool, with no physical sign — and the two can co-occur.

Try this at home

Focus on what your child can do, not labels. Offer short, playful tasks with clear steps and celebrate small wins — this supports attention and confidence whatever your child's profile.

Frequently asked

Can a child have both ADHD and Down syndrome?

Yes. A child with Down syndrome may also show ADHD-type attention or activity patterns. A qualified clinician can look at the whole picture and recommend the right support for both.

Is Down syndrome diagnosed by behaviour like ADHD?

No. Down syndrome is confirmed by a chromosome test, often soon after birth. ADHD is identified by observing behaviour patterns over time across different settings, not by any single test or scan.

At what age can ADHD be assessed?

ADHD is rarely assessed meaningfully before the preschool years, because high activity and short attention are normal in toddlers. A clinician considers whether patterns are clearly beyond what's typical for the child's age.
