ADHD and Developmental Language Disorder can look similar in young children — both may seem 'not to listen' — but they differ at the root. ADHD is about attention, impulse control and activity level: the child understands the words but cannot sustain focus. DLD is about language itself: the child wants to engage but struggles to understand or build sentences, with no hearing, autism or ability cause. They can co-occur, so the key is understanding why a child struggles, not just the surface behaviour. Only a clinical assessment can reliably tell them apart.

Two children may both struggle to follow instructions — but for very different reasons: one finds it hard to focus, the other finds it hard to understand the words.

In short

ADHD (Attention-Deficit/Hyperactivity Disorder) is mainly about attention, impulse control and activity level — a child who is restless, easily distracted, or acts before thinking. Developmental Language Disorder (DLD) is about understanding and using language — a child who finds it genuinely hard to follow what's said, find the right words, or build sentences, despite no other clear cause. The two can look alike (both children may seem 'not to listen' or struggle in a busy classroom), and they can even occur together — so a careful look at why a child is struggling matters far more than the surface behaviour.

How they differ in everyday life

With ADHD, the child often understands language well but cannot sustain attention long enough to act on it — they drift off mid-task, interrupt, fidget, lose things, or rush. Their difficulty is with regulating focus and impulses, not with the words themselves.

With DLD, the child wants to engage but the language itself is the barrier. They may give muddled answers, mix up word order, struggle to follow multi-step instructions, take longer to learn new words, or rely on gesture and 'thing/that' rather than naming. Importantly, DLD is not caused by hearing loss, autism, or low overall ability — it's a specific difficulty with language learning.

Where they overlap: a child who can't follow a long instruction might have ADHD (attention drifted), DLD (language wasn't understood), or both. This is exactly why guessing at home is unreliable — and why a structured clinical observation is so valuable.

When to seek a look

Consider a developmental check if your child by preschool age is slow to talk, hard to understand, struggles to follow simple instructions, OR is markedly more restless, impulsive and distractible than other children their age — especially if it affects play, friendships or learning. There is no single 'test' you can do at home; both conditions are best understood through proper assessment.

Can a child have both ADHD and Developmental Language Disorder?

Yes. The two can occur together, and one can mask the other. A child whose attention wanders may also find language hard, which is why a careful clinical assessment looks at attention, language and behaviour together rather than judging by surface behaviour alone.

Why do both conditions make a child seem like they're 'not listening'?

With ADHD, the child may hear and understand but lose focus before acting. With DLD, the child pays attention but cannot fully understand the words. The behaviour can look identical from outside, so understanding the reason matters more than the label.
