Newborns don't have a cognitive milestone checklist — early thinking shows as alertness: noticing faces, calming to your voice, brief tracking by 6–8 weeks and a first social smile around 6 weeks. A newborn does not have measurable "cognitive milestones" in the way an older baby or toddler does — and that is completely normal. In the first weeks, thinking and learning show up as alertness: noticing light and faces, calming to a familiar voice, and slowly staying awake a little longer to take the world in. There is no checklist to anxiously tick; there is only gentle observation and warm, responsive care.

What is actually appropriate to notice now

In the newborn period (birth to about 3 months), early cognition (ICF mental functions, b1) appears as growing attention and recognition:
  • Alert moments — brief periods of quiet wakefulness when your baby seems to "take things in"
  • Looking at faces — preferring high-contrast patterns and the human face, especially yours
  • Settling to your voice — calming when they hear a familiar caregiver
  • Beginning to follow — by 6–8 weeks, briefly tracking a face or object that moves slowly
  • First social smile — often emerging around 6 weeks, a lovely early sign of engagement

These are signs of a healthy, learning brain — not a test to pass. Babies vary widely, and prematurity shifts timing.

When a check is sensible

Formal developmental assessment isn't meaningful this young. But do raise it with your paediatrician promptly if your baby is very floppy or very stiff, never alert, doesn't startle to loud sound, or doesn't fix on a face by around 2–3 months — these warrant a general developmental and hearing check, not worry alone.
