Signs of cognitive delay in a 5-year-old

At five, cognitive development shows through language, play, memory and problem-solving. Possible signs of delay include trouble following two-step instructions, very limited speech, difficulty counting or naming colours, little pretend play, or learning much more slowly than peers. One sign alone is rarely a worry — it is several areas together, or a clear gap from peers, that makes an early developmental check wise. This is a reason to assess early, not a diagnosis, because support at this age works beautifully.

By five, your child is busy making sense of a big world — noticing how they think, play and solve puzzles is loving, attentive parenting.

In short

A five-year-old shows their thinking through play, talk, memory and problem-solving. Signs that may suggest a cognitive delay include trouble following two-step instructions, very limited speech or vocabulary, difficulty counting or naming colours, struggling with simple puzzles or pretend play, or learning much more slowly than peers. None of this is a diagnosis — it simply means a calm developmental check is wise now, because support at this age works beautifully and the window for learning is wide open.

What to watch at 5 years

Most five-year-olds are curious, chatty and full of questions. Gentle flags worth a clinician's eye include:
  • Language — speaks in very short or unclear sentences, has a small vocabulary, or struggles to tell a simple story or answer "why" and "how" questions.
  • Following instructions — finds two- or three-step directions hard ("get your shoes and put them by the door").
  • Early thinking skills — difficulty counting to ten, naming a few colours, recognising their own name, or sorting objects by size or type.
  • Play and imagination — little pretend or make-believe play, or play that stays much simpler than other children of the same age.
  • Memory and attention — struggles to remember familiar routines, songs or recent events.
  • Self-care — needs far more help than peers with dressing, eating or simple daily tasks.
  • Pace of learning — picking up new ideas much more slowly than children of a similar age, across several areas rather than just one.

A single point on this list is rarely a worry on its own. It's the bigger picture — several areas together, or a clear gap from peers — that makes an early, gentle look worthwhile.

When to act

If you notice delays across more than one area, or your instinct tells you your child is learning much more slowly than other five-year-olds, arrange a developmental check now rather than waiting. With school beginning, an early picture helps everyone support your child well. Trust what you see every day — that is valuable information.

What to watch

Seek a check if your five-year-old struggles with two-step instructions, has very limited speech or vocabulary, can't count to ten or name colours, shows little pretend play, finds simple puzzles hard, or is learning much more slowly than peers across several areas. The bigger picture matters more than any single sign.

Try this at home

Turn everyday moments into gentle thinking play — count the steps as you climb, name colours while sorting laundry, or ask "what happens next?" during a story. Notice how your child follows along; it tells a clinician a lot.

Frequently asked

Is slow learning always a sign of cognitive delay in a 5-year-old?

No. Children develop at their own pace, and one slower area on its own is rarely a concern. It's when several areas — language, memory, problem-solving, self-care — are clearly behind peers that a gentle developmental check is wise. Early support works beautifully at this age.

My five-year-old talks well but struggles with counting. Should I worry?

Strength in one area and difficulty in another is common and often part of typical development. Keep playing counting games naturally and watch how things grow. If the difficulty is wide-ranging or you feel unsure, a calm clinician's review can give clarity and reassurance.

Can cognitive delay be helped at age five?

Yes — five is a wonderful age for support. The brain is still highly adaptable, and early, play-based help builds thinking, language and confidence. A developmental assessment is the first step to understanding your child's strengths and shaping the right support.
