Signs a paediatrician may notice at 3–6 months

Muscle tone and movement
  • Low muscle tone (hypotonia) — baby feels floppy or relaxed when held
  • Slower head control, or arms and legs that seem especially loose

Feeding and growth

  • Slower, more tiring feeds; a little extra patience needed at the breast or bottle
  • Slower weight gain plotted on growth charts

Physical features (present from birth)

  • A flatter facial profile, upward-slanting eyes, a small fold of skin at the inner eye
  • A single crease across the palm, smaller ears, a shorter neck

Development

  • Smiling, rolling and reaching may emerge a little later — every baby's pace varies

None of these alone confirms Down syndrome. Diagnosis is made by a karyotype blood test, not by looks or milestones.

Can Down syndrome appear suddenly at 3–6 months if it wasn't seen at birth?

Down syndrome is present from conception and is usually identified at or soon after birth through physical features and confirmed by a karyotype (chromosome) blood test. It does not appear later, though some families may only have it confirmed in the early months if testing was delayed.

Does low muscle tone always mean Down syndrome?

No. Low muscle tone (hypotonia) has many causes and many babies with it do not have Down syndrome. If you notice persistent floppiness or feeding difficulty, your paediatrician can examine your baby and advise on any tests.

What checks does my baby need if Down syndrome is confirmed?
