The ADOS-2 is a gold-standard, clinician-administered observation used as part of an autism assessment — never on its own, and never something you decide alone. Whether your child should have one depends on what an experienced clinician sees after a developmental review of your concerns. It is a structured, play-based session that watches how your child communicates and interacts, not a frightening test — and it gives the team rich, real information to guide the right support.

What the ADOS-2 actually involves

Think of it as guided play and gentle conversation, not an exam. A trained clinician offers your child a series of friendly, semi-structured activities — toys, pictures, bubbles, snacks, story-telling or chat, chosen to suit your child's age and language. It usually takes around 40–60 minutes.

• It's matched to your child. There are different modules — from young children with few words through to fluent talkers — so the activities fit where your child is.
• It observes, it doesn't quiz. The clinician notices how your child shares attention, uses eye-gaze and gesture, plays imaginatively, takes turns and responds to social openings.
• It's one piece of the picture. The ADOS-2 is combined with a developmental history (often using a parent interview), school or nursery input, and clinical judgement — autism is never confirmed from a single tool.
• You're welcomed, not sidelined. A good assessment explains each step and treats your knowledge of your child as essential evidence.

When it's worth asking for one

An ADOS-2 is usually considered when a clinician has already noted differences in social communication, interaction or play during a broader developmental check — not as a first or stand-alone step. If you're noticing limited back-and-forth interaction, little pointing or sharing, differences in eye contact, delayed or unusual language, or intense focused interests, the right move is a general developmental review first. The clinician then decides whether an ADOS-2 adds value.
