Prematurity-Related Developmental Risk

How Prematurity Affects Your Child's Communication Development

Prematurity can gently delay communication milestones — babbling, understanding, first words and sentences — partly because the brain does key language wiring in the final weeks of pregnancy. Progress should be judged by corrected age (from the due date) until about two years, and most preterm children catch up with time and support. Persistent delays at corrected age, or any hearing concern, are worth an early developmental check.

You watched your baby fight to arrive early — and now you're watching, and wondering, every time other children chatter and yours stays quiet.

In short

Being born premature (before 37 weeks) can gently shift the timeline for communication — first words, babbling, understanding and putting sentences together may arrive a little later than in babies born at term. Much of this is simply because your baby had less time to grow before birth, which is why we use corrected age (counting from your due date, not the birth date) in the first two years. Most premature children catch up beautifully with time and the right support — but communication is one of the areas worth keeping a friendly eye on, because earlier help is always gentler.

How prematurity can touch communication

A baby's brain does an enormous amount of wiring in the final weeks of pregnancy — including the foundations for hearing, attention and language. Arriving early means some of that building happens outside the womb, so communication may unfold at its own pace:
  • Early sounds — cooing and babbling may begin later than expected for the birth date, but usually right on time for the corrected age.
  • Understanding language — following simple words, turning to their name, or pointing to ask for things may take a little longer to emerge.
  • First words and sentences — vocabulary and joining words together can be slower to build, especially in babies born very early or who had a long stay in the NICU.
  • Hearing — prematurity slightly raises the chance of hearing differences, which directly shape speech; this is why newborn hearing screening matters so much.
  • Feeding and oral skills — early sucking and swallowing challenges sometimes overlap with the muscles used for speech.

Remember to always judge progress by corrected age until about two years. A baby born two months early should be measured against a two-month-younger timeline — what looks like a delay is often perfectly on track.

When it's worth a closer look

Reach out for a developmental check if, at corrected age, your child isn't babbling by around 9 months, isn't using single words by 15–18 months, isn't combining words by around 2 years, doesn't respond to sounds or their name, or if your instinct simply tells you something needs attention. Children born very preterm or with a complex NICU journey benefit from regular follow-up regardless.

What to watch

Using corrected age: no babbling by ~9 months, no single words by 15–18 months, not combining words by ~2 years, not responding to sounds or their name, or any concern about hearing. Trust your instinct and seek a check early.

Try this at home

Talk and pause: narrate your day in short, simple phrases and then wait a few seconds, giving your child space to respond with a sound, look or word. This turn-taking builds the foundations of conversation — and always celebrate progress against your baby's corrected age, not the birth date.

Frequently asked

Should I use my premature baby's actual age or corrected age for milestones?

Use corrected age — counted from your due date rather than the birth date — until about two years old. A baby born two months early is measured against a two-month-younger timeline, so what can look like a delay is often perfectly on track.

Will my premature child catch up in speech and language?

Most premature children catch up with time, especially with responsive talking, reading and play at home. Babies born very early or with a complex NICU stay may need more support, which is why gentle follow-up and early help matter so much.

When should I worry about my preterm baby's communication?

At corrected age, seek a check if there's no babbling by around 9 months, no single words by 15–18 months, no word combinations by around 2 years, no response to sounds or name, or any hearing concern. Trust your instinct — earlier support is always gentler.
