Observing energy regulation on a home visit

On a home visit, a frontline worker should observe a child's natural rhythm of energy and drive across the day — alertness for play and engagement, energy to feed and explore, and the ability to settle and rest. Both persistent low energy (listlessness, tiring quickly) and unsettled over-activity are worth noting. This is gentle observation to record and follow up, not a diagnosis; where low energy pairs with feeding or growth concerns, a medical review comes first, then route to a developmental screen.

During a home visit, a child's energy through the day tells a quiet story — and a frontline worker is often the first to gently read it.

In short

On a home visit, observe the child's natural rhythm of energy and drive across the day — how readily they wake, play, feed, rest and settle. You are watching whether the child has enough energy to engage with people and play, and can wind down to rest. This is gentle observation to note and follow up, never a diagnosis made at the doorstep. A pattern that persists or worries the family is best routed to a developmental check.

What to observe (ICF b152 — energy and drive)

Energy regulation is about a child's get-up-and-go and their ability to rest — the balance between alertness and calm.

Daytime energy and engagement

  • Does the child show interest in play, faces and surroundings, or seem persistently listless and hard to rouse?
  • Is there age-appropriate drive to reach, explore, crawl or move about?
  • Or, at the other end, constant restlessness that never settles into focused play?

Feeding, rest and recovery

  • Energy to feed well, or tiring quickly mid-feed
  • Settling for naps and night sleep versus being chronically overtired or wakeful
  • Bouncing back after rest, or staying flat and low through the day

Across the visit

  • A single sleepy moment means little; look for a pattern the family also notices
  • Note if low energy comes with poor feeding, weight concerns or frequent illness — these need prompt medical review, not therapy alone

What shifts this from ordinary variation towards a check is energy that is persistently very low or very high across weeks, affects feeding, play or sleep, or worries the parent.

When to refer

Record what you and the family observe and route to the PHC or a developmental screen. Where low energy pairs with feeding or growth concerns, prioritise a medical review first — anaemia, infection and nutrition are common, treatable causes. Early, kind follow-up never has to wait for a label.
