Observing expressive communication on a home visit

On a home visit, observe how the child sends messages outwards — sounds, babbling, words, gestures, pointing and facial expression — appropriate to age. Watch for cooing and babbling in babies, first words and pointing in toddlers, and two-word phrases by around two years. These are observations to note, not to diagnose. If expressive communication seems delayed for age, or a parent is worried, route the family to a developmental and hearing check, since early support works best when it starts early.

A home visit is a window into how a child reaches out to the world — and expressive communication is one of the clearest things you can gently observe.

What to watch (by age, in the home)

Expressive communication (ICF d3) means how a child sends messages, not just understands them. Watch during natural play and family routines.

Babies (around 6–12 months)

  • Cooing, then babbling with repeated sounds ("ba-ba", "da-da")
  • Reaching, pointing or showing things to share interest
  • Using cries and sounds differently for different needs

Toddlers (around 12–24 months)

  • First clear words by about 12–15 months
  • Pointing to ask for or show things
  • A growing handful of words and gestures (waving, nodding)

Older toddlers (around 2–3 years)

  • Joining two words ("more milk", "go out")
  • Naming familiar people and objects
  • Asking simple questions and being understood by family
