How to apply for your child
- Gather documents — a recent passport-size photo, proof of identity and address (Aadhaar is commonly used), and any existing disability or medical certificate. For children, the parent or guardian applies on the child's behalf.
- Register online — go to the official UDID portal (swavlambancard.gov.in), create an account and complete the application form with your child's details.
- Upload documents and submit. The portal forwards your application to your chosen government hospital or designated Medical Authority.
- Attend the medical assessment — a government medical board examines your child and, where applicable, certifies the type and percentage of disability. This step is done at a notified government facility, not by a private centre.
- Track and receive — once verified, the disability certificate is generated and the UDID card is approved; you can track status online and download an e-copy, with the physical card dispatched thereafter.
The card is renewable for children, as a child's condition is reassessed at intervals the board specifies — so keep a note of any review date on the certificate.
