How to apply for your child

1. Gather documents — a recent passport-size photo, proof of identity and address (Aadhaar is commonly used), and any existing disability or medical certificate. Register online — go to the official UDID portal (swavlambancard.gov.in), create an account and complete the application form with your child's details. 3. Upload documents and submit. The portal forwards your application to your chosen government hospital or designated Medical Authority. 4. Attend the medical assessment — a government medical board examines your child and, where applicable, certifies the type and percentage of disability. This step is done at a notified government facility, not by a private centre. 5. Track and receive — once verified, the disability certificate is generated and the UDID card is approved; you can track status online and download an e-copy, with the physical card dispatched thereafter.

The card is renewable for children, as a child's condition is reassessed at intervals the board specifies — so keep a note of any review date on the certificate.
