Certificate vs UDID
Disability Certificate vs UDID Card: What's the Difference?
A disability certificate is the official medical document, issued by a government medical board, recording a child's condition and disability percentage. A UDID (Unique Disability ID) card is a single nationwide smart card, linked to a central database, that carries this certified information in portable digital form. Both are now issued through one online application. A clinical AbilityScore® and any diagnosis are formed only at a Pinnacle Blooms Network centre under qualified clinician care.
Two documents, one journey — knowing what each one does makes claiming your child's rights so much simpler.
In short
A disability certificate is the official medical document, issued by a government medical board, that records your child's condition and percentage of disability. A UDID card (Unique Disability ID) is a single nationwide smart card, linked to a central online database, that carries this same certified information in one portable, digital form. In short — the certificate is the assessment, and the UDID card is the nationwide proof and access key built on top of it. Today, both are issued through the same online process at swavlambancard.gov.in.How they differ — and how they connect
- Disability certificate — issued after assessment by a designated government medical authority. It states the type of disability (from the 21 conditions recognised under the Rights of Persons with Disabilities Act, 2016) and the percentage of disability. It is the legal basis for benefits.
- UDID card — a single, durable, nationwide identity card generated from that certificate. It removes the need to carry multiple papers across states, and stores records digitally so they can be verified online.
- One application, both outcomes — when you apply on the national UDID portal, you are assessed for the certificate, and on approval both the certificate and the UDID card are generated. Older paper certificates issued earlier can also be linked into the UDID system.
- What they unlock — both support access to scholarships, travel concessions, reserved benefits, aids and appliances schemes, and other entitlements under Indian law.
Think of the certificate as the clinical and legal record, and the UDID card as the single national passport that carries it wherever your family goes.
When to start the process
It helps to begin once your child has a confirmed diagnosis from a qualified clinician, as the medical board assessment relies on this. Keep your diagnostic reports, identity proof, address proof and a recent photograph ready. The process is free and applied for online; the medical assessment happens at a designated government facility near you.The Pinnacle way

What to watch
Keep your child's diagnostic reports, identity proof, address proof and a recent photograph ready before applying, and ensure any older paper certificate is linked into the UDID system so benefits stay accessible across states.
Try this at home
Apply through the official national UDID portal — the process is free, and one application gives you both the certificate and the card, so beware of any agent asking for fees.

Frequently asked
Do I need both a disability certificate and a UDID card?
The certificate is the underlying legal and medical record, while the UDID card is the nationwide portable proof built from it. Today a single online application gives you both, so you do not apply separately.
Is the UDID card valid across all states in India?
Yes. The UDID card is a single nationwide identity linked to a central database, so it can be used and verified across states without carrying multiple papers.
What do I need before applying?
A confirmed diagnosis from a qualified clinician, your child's medical reports, identity and address proof, and a recent photograph. The medical assessment for the certificate happens at a designated government facility.
