The science

Pretend play is a recognised developmental marker because it indexes representational thought — the capacity to let one object stand for another and to hold a symbolic narrative in mind. It correlates closely with theory-of-mind development, receptive and expressive language, and emergent executive control. Reduced or absent symbolic play, restricted play repertoire, and impaired social-interactive play are well-described early indicators within autism and broader neurodevelopmental surveillance frameworks. Conversely, rich pretend and cooperative play is a protective, prognostically favourable sign.

When to refer

Consider a developmental review when, by around 24 months, a child shows no spontaneous pretend play; when play is dominated by lining-up, spinning or repetitive manipulation; when there is limited imitation, joint attention or shared enjoyment; or when play delay accompanies delays in language, gesture or social reciprocity. Surveillance — not single-point judgement — is the appropriate stance: track trajectory across visits.
